Elgibility

For those patients not covered by in-network third party payers or Medicaid, services are billed based on our sliding fee scales.  We accept Aetna, Blue Cross & Blue Shield, Cigna, Medcost, Medicare and all forms of NC Medicaid. Eligibility is determined at each visit, based on the household income supplied by the patient as well as family size.  Acceptable forms of proof of income are W2 forms, Income Tax forms, Pay stubs, Employer Statements and bank statements showing SSI, social security or VA benefits.
